Sword is available to all Active Employees, Pre-65 Retired Employees, and their dependents over the age of 13 who are enrolled in the State Farm Group Medical PPO option administered by BlueCross BlueShield of Illinois.
If you live in one of the 14 states that require a referral for physical therapy, then you will be asked to complete additional onboarding questions posed by Wheel. Wheel will serve as your referral source.
